1. Name of studio and /or Artist
2. Physical address of studio
3. Contact details at studio
4. Pictures of shop, shop front, autoclave, and sterilizing bay.
5. Copy of last invoice of waste disposal and bio waste destruction certificates.
6. Copy of BBP certificate
7. Details of any tattoo artist qualifications
